What is the Spanish PLF?

The Spanish PLF (Passenger Locator Form) is an online health declaration that must be filled out by every incoming visitor to Spain who wishes to enter the country by plane or ferry, including travelers staying in Spain for transit-related reasons. Thus, the Formulairo de Control Sanitario (FCS) is a health control form that can be completed entirely online before entering Spain. How to apply for the Spanish PLF (FCS)?

  • - address in your home country

  • - address of stay in Spain

  • - contact details (such as your phone number and email address)

  • - travel-related details, including your flight number, seat number, date of arrival, the purpose of the planned trip

  • - health-related details (e.g., if you have been in contact with any person infected with Covid-19

What happens after applying?

Once you have submitted your application for the Spanish health form, a confirmation email will be delivered to your email inbox.
